The following links allow you to download our New Client intake forms. Download, print and complete prior to your appointment and bring it with you. This is both convenient and will allow for the maximum massage time.
Certain policies have been implemented for your safety and to insure yours and other clients experience is therapeutic and relaxing. We strive to have a great working relationships and will work hard to give you the best experience possible.
Clients must not be under any sort of mind altering substance.
Inappropriate behavior by any client will result in the session ending.
All clients are asked to have acceptable hygiene.
Communication and Privacy
Open communication is very important to us. If you have any wishes or concerns, please feel free to communicate them.
Confidentiality is taken very seriously. You can be assured that any medical information will be kept safe and strictly private.
Appointments & Cancellations
Appointments can be made on line or by contacting us directly by phone. Once you have scheduled an appointment, that time slot is no longer available to other clients, therefore you are financially responsible for it. No-shows and cancellations made less than 24 hours prior to the appointment will be charged the full price of the service. We also reserve the right to ask for pre-pay of services. Extension of appointment due to late arrival is only possible if our schedule permits.
We accept Mastercard, Visa, check and cash. Payment by check or cash whenever possible is appreciated.
